eth0 does not exist on boot - Slackware

  1. eth0 does not exist on boot

    I'm using slackware 12.1. The eth0 device is not being setup on boot
    for an installed pcmcia card. However, if I manually remove the pcmcia
    card and reinsert it, ifconfig shows that eth0 exists. Also, if I use
    pccardctl to eject and then insert, ifconfig shows that eth0 exists. I
    don't believe I've had this problem before with previous releases of
    slackware. What's the best way (modify init script or udev) to get this
    card to be fully setup at boot?

    I've included my /var/log/messages below, which shows that the card is
    detected at boot, but that the eth0 device is not setup. However, eth0
    is properly setup after I run "pccardctl eject 0" followed by "pccardctl
    insert 0".

    did you: chmod +x /etc/rc.d/rc.pcmcia

    So that the pc card control daemons start on boot?
