    On kernels ranging from 2.6.26 - 2.6.28-rc2 on an Intel P4 3.2GHz HT x86-64
    cpu, I am seeing a bug that occurs about half the time when copying ~300MB
    data from a usb flash drive (FAT) to hard drive (XFS). There are three
    possible outcomes, so far, when I hit the bug:

    (1) The copy process hangs, cpu wait hits 100% and load avg climbs until
    reboot;
    (2) The copy process hangs and then recovers and then one of:
    (a) the cpu wait & load avg both decline to normal; or
    (b) cpu wait hovers at 100% and the load avg slowly climbs until reboot;
    (3) Kernel bug reported and machine locks up.

    Result (1) is the most frequent. Result (3) happened only twice out of about 40
    tests. I lose the full traces, but here are two relevant lines that I see on
    screen before the machine locks up:

    When (1) or (2b) occurs I pull the usb drive to see what happens. In two cases
    (3) occurred, but more often I get this in /var/log/syslog:

    In theory, this sort of thing could have been caused by changes to the
    block-layer timeout handling in 2.6.28-rc. But those changes are not
    present in 2.6.26 or 2.6.27.

    Note that the syslog shows a lockdep violation occurring _before_ the
    error message from usb-storage. This suggests the problem originates
    somewhere else.

    The error message:

    means that the SCSI layer has told usb-storage to start a new command
    before the old one completed. This could happen, for example, if the
    SCSI layer had gotten confused and tried to cancel a command that
    wasn't running -- then it would think usb-storage was idle when in fact
    it was still busy. (This was the failure mechanism resulting from the
    timeout changes.)

    Brian, if you can use usbmon to monitor what happens during the file
    copy and subsequent error, it might provide a clue. Instructions are
    in the kernel source file Documentation/usb/usbmon.txt.

    Yes, that's the correct line, it was 2.6.28-rc2 that produced those.
    I'll try usbmon per Alan Stern's suggestion and see if that shows
    anything interesting.

    This is exactly the failure mechanism I described earlier. The usbmon
    log shows that a communications error occurred and usb-storage tried to
    carry out error recovery. The drive apparently did not reset itself,
    because when the READ was reissued no data was received. In fact, the
    log _ends_ at the point where the READ was reissued; no more data was
    transferred in either direction. Presumably the next thing to occur
    was a command timeout.

    And since the block-layer timeout code in 2.6.28-rc2 is known to be
    buggy, it's not surprising that the copy process hung at that point.
    It would be better to run this test under 2.6.27; that way we might
    learn something new.

    I tested with 2.6.27.5 and found that, although the process would hang as often
    as usual, it always recovered instead of needing to reboot. No kernel bug or
    system freeze, no climbing load avg, etc. Here is the usbmon output on a

    This is essentially the same failure mechanism as before, but without
    the timeout-related kernel bug.

    There is a communications error during one of the reads. It takes the
    same form in both logs: A transfer receives only 4051 bytes when it
    should get 4096. Don't ask me why that happens; it's some sort of
    hardware or firmware failure either in the drive or in your USB host
    controller.

    The kernel tries to recover, but it looks as though the drive is stuck
    trying to send the remaining bytes. Resets don't help, so the drive
    is taken off-line.

    You _might_ be able to prevent these problems by reducing the drive's
    max_sectors value, say to 128. See
