After a recent reboot, fax send is now working normally.
The device files list as follows, so the problem must have been
ownership rather than permissions:

lr-xr-xr-x 1 root root 10 Nov 2 12:35 /dev/modem -> /dev/ttyS0
lr-xr-xr-x 1 root root 5 Nov 2 12:35 /dev/ttyS0 -> tts/0
crw-rw---- 1 adam tty 4, 64 Nov 3 17:11 /dev/tts/0

Actually my system _does_ use devfs, but it must have got that device
wrong at some point. Any ideas on (mis)configuration?
